5-MTHF Folate (L-5-Methyltetrahydrofolate, Quatrefolic®, 1.2 mg = 2 mg DFE per dose) is the biologically active form of folate, which the body can use directly without the need for metabolic conversion by the MTHFR enzyme. This is particularly relevant for people with genetic variants that reduce the activity of this enzyme (C677T or A1298C variant of the MTHFR gene), making conventional folic acid supplementation less effective. Folate is an essential cofactor in the remethylation pathways of homocysteine to methionine.

Vitamin B12 (as Methylcobalamin, 1.2 mg per dose) is the active form of vitamin B12, which the body can use directly as an enzymatic cofactor. Methylcobalamin acts as a methyl group donor in methylation pathways, being essential for normal homocysteine metabolism, nervous system function, and normal red blood cell formation.

Vitamin B6 (as Pyridoxal 5'-Phosphate, 20.4 mg per dose) is the active coenzymatic form of vitamin B6, an essential cofactor in the transsulfuration pathways of homocysteine — the alternative pathway that converts homocysteine into cysteine and taurine, amino acids beneficial to the body. This active form does not require hepatic conversion, and may be more effective in contexts of methylation support.

Anhydrous Betaine (Trimethylglycine, TMG, 1.8 g per dose) is a natural compound present in foods such as beetroot. It functions as a methyl group donor independently of B vitamins, serving as an alternative pathway for remethylation of homocysteine to methionine — especially relevant when folate-dependent pathways are compromised.

Important considerations

  • Interaction with methotrexate: 5-MTHF supplementation is not recommended concurrently with methotrexate in oncological chemotherapy, as it may interfere with its antineoplastic action.
